The Northern Valley Health Coalition is hosting a presentation from Sunshine Community Health Center’s Director of Health Equity and Social Determinants of Health Services at 10am on March 12th via Zoom. The presentation explores what social determinants of health are, how they affect your health outcomes, and what Sunshine Community Health Center is doing to lead the way in addressing health disparities in the northern valley communities.
